INJURIES

CFA Injury Reporting Procedure -Every effort should be made to prevent any training injuries. CFA Safety Policies are designed to minimize the potential for Training injuries. However, at times injuries do occur and we must be sure to report all injuries so that we can revise policies to prevent similar occurrences in the future. All paperwork must be submitted directly to Alan Zygmunt

CFA has two separate procedures to report injuries; Student Injuries and Employee Injuries

For Student injuries, have them fill out the student injury form completely. Be sure to secure Medical care if warranted.

For Employee Inquires there is a more extensive procedure. Employees should follow the Injury Reporting Check sheet to ensure all required forms are completed and forwarded to the Safety Officer. Also shown below is an Information Brochure provided by the Workers Comp Administrator.
